1,4-butanediol is fairly different than GHB and GBL. Many people report much more dizziness and other side effects. There are also very serious concerns about the safety of 1,4-BD, and there is some reason to believe that it could be involved in damage to the liver, kidneys, and circulatory system. It is not recommended as a GHB substitute, and I don't consider it safe for human use.

Erowid has received more reports of negative health effects and unpleasant side effects from 1,4-Butanediol than we have from GHB or gamma butyrlactone. Whether this is the result of 1,4-B being more toxic and/or more likely to contain impurities, or merely the result of our small sample size is impossible to say. There is very little formal data or research which looks at comparative risks of GHB, GBL, and 1,4-B.
